Sunrise Children's Hospital Now Offers Online Pre-Registration
Who can Pre-Register Online?
- Patients with a procedure or surgery already scheduled
- OB Patients who would like to pre-register for labor and delivery
How to Pre-Register Online
- Complete the 10-20 minute Online Pre-Registration and Mammogram Scheduling forms at least two business days prior to your scheduled appointment.
- For privacy reasons, if you leave your computer idle for more than 20 minutes, your form will not be saved and you will have to begin again.
- Check-in with our registration staff on the date of your scheduled service to complete the registration process. Please be advised that you will sign all consent forms during your visit.
Before you begin, be sure you have all of the items listed below on hand:
- Social Security Number
- Name of ordering physician
- Date of your scheduled procedure
- All insurance card(s) information
- Employer information

Prefer to register online or in person?
The Women and Children's Admitting Department is adjacent to the Children's Hospital Lobby on the south side of the facility. The Women and Children's Admitting Department is open 24 hours per day 7 days per week.
The doors to the Children's Hospital Lobby are open from 5:00am - 10:00pm.
For Admitting Services from 10:00pm - 5:00am, please enter through the Emergency Department on the South side of the hospital.
The Women and Children's Admitting Department coordinates Pre Registration/Registration for all obstetric services, direct pediatric admissions, pediatric infusion and blood transfusion services as well as other outpatient services performed at the hospital. To expedite registration and ensure proper billing, please bring all insurance information, photo identification, and any applicable co-payment or deductible at the time of registration. Sunrise Hospital Admitting
The Admitting Department is adjacent to the main lobby on the north side of the facility. It is open Monday - Friday 6:00am - 8:00pm. To expedite admission and ensure proper billing, please bring all insurance information and photo identification at the time of admission. For more information contact Admitting at (702) 731-8126 option 1.
For Admitting Services from 8:00pm - 6:00am, please enter the Emergency Department on the south side of the hospital. The doors to the main lobby are locked after 10:00 pm each night.
The newly renovated Pre-Admission Center has re-opened at its original location inside the Diagnostic Center Building at 3006 S Maryland Pkwy, Suite 350.

This center processes all surgical patients having procedures in our operating room, cath lab and special procedure units. Pre-admissions are by appointment ONLY. Please make an appointment by calling (702) 304-3151 and selecting option 1. Prior to your pre-admit appointment, please be sure you have completed your physician visits and have orders for the services to be performed. We are unable to complete the pre-admission process without these orders. Please bring a list of current medications that you are taking, picture identification, insurance cards, forms and your co-pay and/or deductible.
You should allow approximately two hours to complete this process, if no preoperative testing has been completed prior to your visit.
Once you have completed your preadmission process you will be directed to the Surgery Waiting area on your day of surgery and will not need to return to admitting.
Enter through the door for ambulatory patients. The Emergency Department is open 24 hours per day 7 days per week. To reach the Pediatric Emergency Department call (702) 731-8181.
Sunrise Specialty Services
This building is located at 3131 La Canada #107. This is on the east side of the main hospital. The entrance is located on the southwest side of the building. The entrance hallway will take you directly to registration. Hours of operation are Monday - Friday 7:30am - 5:00pm, Saturday 9:00am - 12:00pm, closed Sunday.
A registration representative is located in the surgery waiting area. Hours of operation are Monday - Friday 5:00am - 1:00pm.
This building is located at 3006 S Maryland Pkwy. Enter through the door on your left, which takes you into the lobby of the Diagnostic Center. Hours of operation are Monday -Friday 7:30am - 4:30pm. All of the outpatient laboratory work for Sunrise Hospital is performed in the Diagnostic Center. If you are scheduled for lab work only please go to this Center.
The Breast Center at Sunrise
The Breast Center is located at 3006 S. Maryland Pkwy suite 201. Enter through the main door and take the elevator to the second floor. Hours of operation are Monday - Friday 8:00am - 4:30pm.
Inpatient Registration Information
Your physician will call our Operations Center if you need to be admitted to the hospital. The Operations Center will arrange for your room. Room assignments are made based on your medical needs. Private room requests will be honored if the room is available. Private rooms cannot be reserved or guaranteed.
You should go directly to the main admitting department when you arrive at the hospital. Please remember to bring your insurance cards, insurance forms, picture identification, co-payment and/or deductible, and, if applicable, your Advance Directive.
Outpatient Registration Information
We register patients and perform services based on your physicians order. This order must be complete or it may postpone or delay your services.
The information required on your order is:
- Patient name
- Procedure being performed/ordered
- Diagnosis or symptoms (can not have a rule out or possible)
- Signed by your physician
If this information is not available at registration we will need to obtain this information prior to registration. Please review these requirements with your physician when the order is being prepared.
You will need your insurance cards, insurance forms, picture identification, and copay or deductible at Registration, and if applicable, your Advance Directive.
